If you need to file a claim, log into My Account, then download and fill out the required form. The completed form along with all required documentation can be submitted by email, fax or regular mail. Our commitment is to pay approved claims as quickly as possible and we strive to make claims decisions and fulfill approved reimbursements within 5 days of receiving the required and complete claims form and documentation.
